Hi, my son Kieran is almost 7 mo, was diagnosed with AR when 3 mo. He's been on Zantac & now Prevacid. We've regularly wondered if he has something else causing or exacerbating the AR. From birth, he strains/grunts to have bm, which occur 3-5+ days, though he's bf. When he passes stool, it's always soft, not constipated. Our ped told us this would work itself out but hasn't.. We started solids (rice cereal, then oatmeal), which seemed to make things worse, so we backed off (but are slowly trying pears/peaches again). (Otherwise, he's exclusively bf.)
He often spits 1-3 times between feedings (roughly every 3 hrs), sometimes up to 2 hrs past a feeding (then vomit is yellowish and curdled). Occassionaly spit is projectile a few feet. Spitting up seems to correlate with grunting/straining to pass gas, work on stool (which is quite often during day and at night).
The odd part is that one would expect him to be relieved by passing gas or stool. Instead, the night before he stools, he's calmer and sleeps better. The night after he goes is the worst in the cycle. Today 20 min. after a BM he started straining, grunting, and spitting up a great deal again. His gas & BM are both foul-smelling.
We saw our ped again today because we feel instinctively something else is wrong. He kept assuring us all was normal until he learned of the foul-smelling stool. Now he put Kieran on lactulose for a week, and then says if it doesn't help, he'll order a GI test.
Kieran seems too old for "uncoordinated pooping," and he naps/sleeps horribly. He goes to sleep quickly but wakes uncomfortable, though we've taken all the steps for AR (medication, elevation of head, etc.). Plus, he doesn't wake screaming. He rarely cries much, just seems uncomfortable most of the day.
Do you think it's possible that some type of minor obstruction could be causing his problems? How often does such a case occur? Our ped seems to think it's unlikely, but we feel certain something more than AR is going on here.
