Hi Dr Olson, thank you for your time,
I just have some questions....I am sorry this is a bit long...
Naomi was born by ceasarian at 37 weeks and at birth presented with choanal stenosis and wouldn't feed this was dialated at 4 months. It took her a few weeks to learn to breastfeed and she vomited from the word go. She has had an upper Gi at 6 weeks which showed reflux, she was clear on the pyloric stenosis ultrasound, she had a barium follow through which showed slow motility and she had stomach xray which showed constipation. She also had a barium enema which showed a redundant sigmoid colon. The barium enema also showed that her stomach lies horizontal instead of vertical and that is probably why it has trouble emptying. She is now 10 months old. She used to vomit "everything" up to 4 hours after a feed and had a lot of trouble gaining weight, but now since starting her meds, she is finally gaining, but slowly. She is interested in solids, but can only handle a small amount or she will vomit or reflux. The thing is that at about 7 months the vomiting stopped and she just possited every now and then, but she continues to reflux all day long. you can hear it going up and down. She also had a slight wheeze every now and then after feeding. She is still breastfed. She has always been constipated, even when she was born she didn't poo until about 35 hours later. She has had a rectal biopsy to exclude hirshbrungs disease and it was clear. They weakened the muscle and she has been alot better since. I used to have to use suppositries, now I don't. The problem is in the last month she has started projectile vomiting and vomiting her solids. She also gags and chokes on liquid and food. She is seeing the GI at the end of the month and he is going to do a scope to see if there is any damage and what is causing her problems. I guess I can get overwhealmed by all of this information, but I was wondering if you have seen many children with redundant bowels and if this is a major issue with motility problems and what do they normally do to fix this? Also, is her stomach abnormal to lie this way and would a CT Scan be relevant to determine why this is. They think the transverse colon pushes it up, but I am not sure why. She has always been a very relaxed happy baby, but is still not crawling, but sits and rolls around. She is currently on 20mg losec, 0.5ml erythromycin, and 25ml duphilac a day. Her poo is still hard like adult poo too. She is currently 10 months and weighs 8.16kg. and is 65cm tall. I am sorry this is so long, I just want another opinion on how you would manage this situation.
