I have a question about the timing of meds. I am wondering if all the meds I give my 6 wk old refluxer have to be 30 min before meals. He's breastfed approx every 2 to 2.5 hours. I hate the thought of having to wake him up to give meds then wait 30 min to feed him, I know he will be screaming. That will give him more gas, more reflux and so the vicious cycle continues.

He is on the following meds, all of which I think have to be given 30 min before feeding. Can you confirm that?

2.5ml of Losec once a day
1.5ml of zantac twice a day
.5ml of domperidone 4 times a day

Can I give the zantac and domperidone together? Can I give anything AFTER a feed or is it not as effective?

I know to give the Losec by itself and nothing else for 1 hour.

Thank you for any help I am quite lost and distressed.

Hi Annabel, while it is true that the omeprazole (losec) works better when given 30 minutes before a meal and you should strive to give it that way, it will still work relatively well if given with a meal. Only for continuously fed kid on gi tubes do they recommend stopping feeding for 2 hours then resume feeding 1 hour after losec. The domeperidone and zantac can be given together with a meal. The domeperidone keeps everything moving down so it works better when it can be absorbed before the meal. However if you are nursing that frequently and giving it 4 x daily then it is probably going to be working with each feed even if you give it with the feed. In a nutshell, to keep things from looking impossible for you, you could give all three together and have them work relatively well. Ideally, losec 30 minutes before feed, domeperidone 15-30 minutes before feed and zantac at the same time.
