Hello;

Let me start by thanking you for being on this site- I am so grateful.

I live in Montreal and the healthcare here leaves something to be desired. I diagnosed Maiya with reflux, as her pediatrician insisted it was only colic. It took me four months to find another pediatrician who would take us. He is fantastic, but since he took us on with an already exploding caseload, he has availabilities only once every 3 months.

He ordered a ph probe when maiya was 6 months, which revealed severe GER. She refluxed 219 times in 24 hours, with many episodes lasting over 17 minutes with ph of 1.1. Hence, her Boix Oschoa score was 40.

We only see the GI every 6 months, due to the horrible healthcare we have.


Maiya is now on 30 mg prevacid and 30 mg of zantac. SHe is 14 months old and still wakes up every hour, crying at night. She rubs her nose constantly, maybe while refluxing. I find her face swollen a bit. Every morning, I see her as puffy under the eyes (not in the orbit, but under it , where the zygomatic and maxillary bones meet-- if you will, on the lateral aspect of the maxillary sinus, if my memory serves me right. It is yellowish , green, kind of the colour of a hematoma or inflammation. She has had several ear infections too.

I keep thinking allergies or sinus infection, but since there is no green drainage and no fever, no one takes me seriously.

She was solely breast fed for 9 months (i was on a total elimination diet) and now is being supplemented by alimentum. We are having a very difficult time introducing solids at 14 months, so I dont think food allergies are a problem


Could she have chronic sinusitis despite her lack of symptoms. If so, how can i get the pediatrician to consider this.

Hi,

I'm Dr. Olson, a pediatrician in Michigan who has an interest in GERD and reviews the statements on this wonderful site for medical validity.

I would wonder about a couple of things. First, allergies indeed may be a possibility in this scenerio and a useful and safe test might be to try a daily 2.5 to 5.0 cc dose of loratidine liquid, which I suspect is OTC in Canada, to see if the nasal symptoms go away. Allergy testing is tough in this age group and a theraputic medicine test might be easier. This is a safe medicine.

Sinusitis is rare in a child this age, with poorly formed and aerated sinus cavities. It is possible, but I think allergy would be at the top of my list of problems to explore.
