View Full Version : Retched passed fundo!!!
VTC1028 03-03-2006, 09:09 AM Oh no, I think Tucker retched passed his fundo. I'm not 100% certain though. We were talking a walk this a.m., and I looked down and he had spit-up all over the front of his shirt and down the side of the stroller. I never heard him retch though, which is very odd. It looked mucusy w/ some formula in it. Since it had formula in it and he doesn't eat anything PO, that means he retched passed his fundo, right? (B/C the formula had to have come up from his stomach, right???)
BUT, he just retched really badly again and nothing came up through his mouth, he just shot it all up the syringe. The difference between this time and the time I think it came passed his fundo is that he was attached to the pump that time, so it would have had to be violent since he can't vent/retch with that going. Does that make sense to you guys? Sorry I'm babbling, I'm just freaking out here.

I don't know what to offer as I don't have experience with this, but is there a test they can do to see if the Fundo is still working properly???

No, it didn't come from the syringe the first time b/c he was hooked up to the pump. I called the doctor and am waiting for him to call me back :(. I think they have to do an endoscope to see if it's still intact, but I'm not sure (Does anyone else know?).

They can see if it is intact through an upper gi with a qualified tech experienced in reading pediatric upper GI's wih fundoplications. Notice the specific qualification here! If not, an endoscopy is the next route. Not all docs will do that right away though.
It is very possible that he retched past it, continued retching will cause the wrap to break down and not work well. Sorry to be the bearer of bad news.
Did he have a hiatal hernia before? Another possibility is that the wrap is intact but he has another hernia. This would go through the wrap up into the diaphragm forcing the wrap to loosen and allow contents up.
I would definitely call the GI today, this is a big deal. How long ago did he have it? I don't remember.

I have three kids that have had fundos and two of them never retched or vomitted passed their fundos. My dd has been able to since about 6months post op. I do know that her fundo is still intact and she is not refluxing near what she used to in the past.
I would still call your doc and let them know that he did vomit past the fundo to get their thoughts on it. My oldest son just vomitted for the first time in over 6years past his fundo a month ago. I did talk to the gi doc and we think that he had a bug that caused it. We have not seen any of this since.
Does he seem sick to you? I was also reading when he is on the pump that you cant vent. Have you ever tried using the farrel bags because they can be on during feeds, they are a gravity bag to help with venting. Some docs dont recommend using them at the same time because some kids wont hold anything in. We have used them with my son.
I do hope that this is a one time thing. Wishing you the best.

ElisMom 03-03-2006, 12:40 PM Could he be sick or have some gas that pushed past the fundo? I wouldn't worry too much, it could be a freak incidence. Some stuff from his tummy could have shimmied through (if thats a word). I would be more concerned if he forced it up, if it just appeared he could have had a bubble get through with some spit up. I would call the doc and let him know but don't panic! Easier said than done!

No he didn't have a hiatal hernia before. Could he have one now? He had the fundo/g-tube surgery in August.
I'm STILL WAITING for the GI to call me back. UGH. He usually calls me right away, but Fridays are his 1/2 days....I"m hoping his new secretary got him the message in time!!!

thanks so much. I'll try to relax!! Yep, this is the 1st time he's done it. He doesn't seem sick. Kendall was sick earlier in the week--although she had a fever and was vomitting and Tucker doesn't have a fever. Maybe she passed it on to him and he just doesn't have the fever yet.
I've never heard of the farrel bags. I'm definitely going to ask about them. Venting has always been a big issue for us because Tucker is extremely gassy. Every time he retches he shoots up tons of gas, so we think the gas has something to do with the retching (making his stomach too full maybe)?
Thanks so much for your kind words everyone. I'm trying to remain calm. He's retched a couple more times, but I've been right there to vent him and nothing has come up through is mouth. I'm hopeful it was a 1x thing.
VTC1028 03-03-2006, 01:17 PM Okay, GI called back finally. He said not to worry too much if he only did it once. We'll just keep an eye on it. Since we are doing well weight-wise, he said to turn down the pump to 30 mls/hr for a couple of hours to give Tucker's stomach a break. Phew. I feel much better now. I'm just hoping it really was a fluke. Thanks for all of your support. It's great to know I can turn to you guys--it really helps.
Carla 03-03-2006, 02:50 PM I am glad he didn't have a hernia before. He is less likely to slip the wrap. Kids that have trouble (in my experience) with fundos slipping are those who also had hernias. It is good to know if something has to come up, it can. That is a sign of a good fundo. Hopefully it was a fluke and it will get better.
Have you asked about meds to control the retching??
ElisMom 03-03-2006, 03:00 PM :smt047 Glad you heard back from the GI, retching will cause more retching, so I wouldn't be suprised if it continues a while, turning down the pump is a good idea, the more we feed Eli when retching the worse it gets, as soon as we give the tummy a break, he will begin to get better. Hope the same for you!
VTC1028 03-03-2006, 04:41 PM that's how it usually is with Tucker, too. Give his tummy a rest and he recovers. But as soon as we up it again, the retching starts again. I can't wait until Tuesday and our appt at Shands (Univ. of Florida's children's hospital). It can't get here soon enough.
Of course, as soon as I hung up the phone with the GI, Tucker vomitted past fundo again. :( He's acting very lethargic, but I'm hoping he's just worn out from the retching--still no fever.
Poor little man.
Oh, we tried Zofran for the retching--started 3 days ago--it did NOTHING. Actually we thought it was working the first night, but I think that was a fluke that he didn't retch because since then he's been retching just as much as normal. Which is way too much--about 10-12 times a day--violently. I've noticed that he retches more now that he's sitting up (YAY to sitting up, BOO to retching). GI said we could hypothesize (sp?) that he has scar tissue that is pulling when he sits up and that is making him retch. He just wanted to watch it and see, and see what Shand's comes up with.
ElisMom 03-03-2006, 04:44 PM If he continues to feel lethargic or feels any worse I would give them another call or take him in. With all the retching and/or vomiting past the fundo it could indicate something more serious. And, if it continues it could blow the wrap.
VTC1028 03-03-2006, 07:15 PM I talked to the GI again. He said to go ahead and give him pedialyte for a few hours to give his tummy a total break but still keep him hydrated and then up him to 20 cal formula for a few feeds and then to 24 if he tolerates that.
Of course, I went out to get the pedialyte when my hubby got home and Tucker started acting perfectly normal! UGH! I swear sometimes I think I'm nuts--or that someone is going to think I have Munchausen's by Proxy (or whatever that disease is called.....where the parents intentially make the kids sick)

melba19 03-04-2006, 11:07 AM Boy Victoria do I understand. I have actually been accused of that but was unfounded. With Justin no one could explain why he was so sick so they thought that I was the cause but not. To this day I am paranoid when dealing with docs and dont trust any of them.

VTC1028 03-04-2006, 01:39 PM He did great all night for my hubby--who had Tucker duty. Then of course first thing this morning, when I took over, he started retching again. Is he allergic to me??!!!! He's not as bad as yesterday, but still retching some. He's teething and drooling a lot, so I'm thinking all that drool has something to do with it, but who knows.

ermom 04-26-2006, 05:31 PM My little guys wretched past his 1st fundo 11 weeks after it, when he got the stomach flu and basically dry-heaved until he was hospitalized. It was very evident after that illness that the fundo has been hernaited again. His eating and sleeping habits changed, he started gagging and refluxing very audibly. He stopped wanting to eat, struggled to gain weight, etc. I think you'll start to notice some signs if it indeed did come undone. I hope you got some answers by now from your gi. But just know that your instinct here is the best one!! If it hasn't happened again, it was probably a fluke. But trust your instincts. You know him better than anyone else.
My best advice is just to breathe deeply, have a glass of wine, and to watch him closely! In time, you will be able to tell if it's intact or not!!

Hannah got really good at wretching and vomiting despite the fundo. We had test's and her fundo was still in perfect place.
they also thought maybe she had a hernia(from the surgery) That was good also.
Well she kept throwing up and the couldn't figure out that th problem was. They next plan was to put in a GJ tube. They were going to do it right then and there but the test to see iff she needed it came back perfect!
So we went back to smaller feed's over longer time's for a while. Slowly we were able to adjust it.
It has now been almost 2 month's and she hasn't thrown up or retched. Her reflux isn't bad enough right now to even need med's.
She is also eating really well and we are barely using the g-tube.
I hope with time it will all get better for you too! I think it just took Hannah a really long time to recover from the surgery.
